All patients are registered with the practice not an individual doctor. Complete and sign a medical card or registration form (a questionnaire must be completed at the time of registering to be fully registered with the practice) You can either download or complete the online forms.
Acceptance on to the list is as at the discretion of the Doctor. We may also ask you for proof of identity. (Please refer to our registration policy below and why we ask for this).
Your medical records may take several weeks to arrive at the Practice.

There are a number of reasons why you may not be able to register with your chosen GP. For example, you may live too far away. If this is the case simply choose another GP in your local area.

We are currently NOT routinely accepting new patients. Registrations are only accepted for new babies with a parent currently registered at the practice and anyone moving into the same address as a patient currently registered.
To register you’ll need to complete a registration form. You can download this registration form, complete it, and then email it to [email protected].
